To complete your survey online, you need your unique Survey Code from the address label on the paper questionnaire or letter you received in the mail.
The survey code you entered is not valid. Please make sure that you typed the survey code in correctly. Sometimes characters are hard to decipher, or are easily misread. For example, a ""Q"" might look like ""O"".
If you continue to see this screen after you correctly entered the Survey Code from the mailing label that was sent to you, please call Customer Support at 1-800-727-9540. When calling, please provide us with your survey code and the name of the survey.
